關於成績單及學位證明的問題

彌封方式

1.帶已準備好的信封到學校櫃台（是否需先書明各項資訊於信封上，視各人及各申請學校而定）
2.一次申請所有文件
3.當場折好放入信封，先別封起來
4.拿給承辦人員確認裡面沒有放簽名照、美金之類的東東
5.當承辦人員面把信封黏起來
6.拿給承辦人員於彌封處蓋單位章（遇到懶一點的還會把章直接拿給你自己蓋）
7.必須直接寄的自己拿去寄

ps.有些學校會要求一併放入transcript request form（雖然我不知為什麼），請自行確認各校規定

東吳則是一份三十元(聽說的)

我來補充一下吧
東吳的中文跟英文成績單都是一份30元
中英文的學位證明書則是一份10元
但是中文學位證明書必須自行攜帶畢業證書正本到學校
註冊組的人會幫你印好
以上每一種都是個別彌封好的
申請後須二至三個工作天後才能拿

以上報告完畢

那是一張申請表格，填完後向原畢業學校申請成績單用的（有些美國學校要這樣才發吧？） i60

是一般航空信封?還是跟學校要正式的信封?

Any document not received in this manner is considered unofficial. (Signatures of a professor, government officials other than Ministry of Education, and/or notary public are NOT accepted as official.)
這是我想要申請的一所學校中要求,劃線的部份表示一定要用印學校表頭的信紙
但or後面的意思是?如果沒有用信紙可以用蓋的? ;-S

如果請老闆寫,就要用印有公司名稱的信紙,
如果請老師寫,則要買學校或系上專用的信紙
只要不是用institutions letterhead or stamped with the official logo/signatures的一律視為非正式文件

INTERNATIONAL "OFFICIAL" DOCUMENTS
Documents submitted to the Graduate School must be written in the language used for instruction (original language). An English translation of all documents submitted must also be included. Documents must be prepared by the institution which prescribes the curriculum of study.

"Official documents" are defined as official records of transcripts and degree statements in original language AND the English translations prepared by the institution. These documents must be sealed in the institution's official envelope with the official stamp across on the envelope. The individual pages of all documents must be printed on the institutions letterhead or stamped with the official logo/signatures of the proper authority. Any document not received in this manner is considered unofficial. (Signatures of a professor, government officials other than Ministry of Education, and/or notary public are NOT accepted as official.)

沒錯！他指的是成績單！
那你就別擔心，一般學校發的成績單都會符合要求的！記得彌封就好！

倒是我用紅色標的部分，應該是指你的學位證明（畢業證書影本or學位證明書，影本記得要有教務處蓋章）
我知道成績單上通常有列明degree conferred，但有些學校會堅持要學位證明
mail or tel 去問問
不想麻煩就連成績單一起彌封去吧（上文沒看到分開彌封的要求）

頂多再加上生日，應該就可以了
如果覺得不會太雜亂，也可加上自己住址（有些學校倒是有要求）

不過要注意的是
有些online system如果你要查詢忘記的密碼時
會要求你輸入生日
那你就不要把生日寫在信封上喔

可以先去查詢密碼的網頁看看它要求輸入的資料是哪些，不要跟它完全重複
這是保護自己資料的方法喔
