不用制式表格也是可以的，
rec. form第一頁中間就有提到了。
"If you would prefer to write a letter, either in addition or in place of this form, please feel free to do so."
不過既然都找到這個form了...

申請Cohorn MBA才是附件第三頁的那個吧~

據我所知 UTD至少有兩款MBA

一個是bonbon的cohort（MBA資優班），

另一個是幾百人的professional MBA（大拜拜班），

大拜拜班的要寫Narrative而不是cohort的essay......
