根據追夢網的解釋(針對AC差異)
A中的第一個it是虛主詞, 替代的是that後面所接整個句子
第二個it才是指schistosomiasis
so that / so as to 句意都差不多的 ...A和C都是對的選項
刪除C的原因是, 原題的句型it is........that .....已經符合英文慣用的語法[由輕到重,即把句子的重心放到句子尾部]且又是強調的句型,所以不需要使用倒裝句型,而C是倒裝句

上莫老師課解這一題時，他說第一個it指的是schistosomiasis，但沒說第二個it代誰。
記得後來有一次上課他又說一個句子中如果有兩個代名詞，兩個代名詞所代替的是相同主詞（才不會ambiguity)，
所以這兩個it應該都是指schistosomiasis。

應該沒有說錯吧... :^)

這題如果以 althogh 對稱結構來判斷, although S + be ..., S + be ... => (A) 會是較佳選項.

OG 解釋:
Choice A is best: is links the noun schistosomiasis with its modifier, debilitating, and so debilitating that idiomatically introduces a clause that provides a further explanation of debilitating.
C uses an awkward and wordy construction in place of a that... clause.

先前對這個句子的理解好像不太對. (A) 的兩個 it 應該都是指代 schistosomiasis.
後來發現其實補習班所盛行的說法:兩個主詞用 so...that... ,一個主詞用 so...as to 並不完全正確.
so...that... 和 so...as to 的區別我覺得在於語意的差異. so...that... "多麼...使得..."意在感嘆(重心在前面的 so...); so...as to "如此...以致於"表示目的(重心在後面的 as to...).
所以 (C) 事實上扭曲了句意;另外, although 需注意子句和主句的結構對稱性,倒裝破壞了對稱性.
